What to expect
On your first visit we'll show you around the office, go over your health history & health goals, and take any necessary x-rays.
If you have a specific health concern, our goal is to figure out what is causing the problem and give you a plan to correct it.
We will call the number on this form to confirm your appointment time and provide directions (if necessary). Your paperwork will be sent to the email listed on this form. It can be completed right from your phone or computer and submitted back to us online.
